Heart to Home Coordinated Entry

Coordinated Entry is not a housing program — it is a system that assesses your needs and matches you or your family to available resources across the community. Take the first step by completing a coordinated entry assessment so we can connect you to available housing and support options that fit your needs.

The coordinated entry process is designed to get you connected to the right program as quickly as possible.

Five Steps From Crisis to Housing

1. Get Assessed

Call 254-297-8929, walk into an access point, or connect with a street outreach worker. A trained assessor will meet with you to understand your situation.

2. Get Prioritized

When housing openings become available, programs pull participants from the Priority List based on need and vulnerability — not first-come, first-served.

3. Get Matched

Based on your situation, you'll be referred to the right pathway: Victim Services, Homelessness Prevention, or the Heart to Home Priority List for housing programs.

4. Stay Connected

Once on the Priority List, call monthly to leave a check-in message confirming you're still in the region and still need assistance.

5. Get Housed

The housing program (not Heart to Home) will contact you directly when you're matched to an opening you're eligible for.

Note: Waco Housing Authority programs (vouchers, public housing units) maintain a separate waiting list. Contact them directly at 254-752-0324.
