Join the Heart to Home Coordinated Entry System (CES) to coordinate referrals, assessments, and data through cohesive systems.

Join the Homeless Management Information System (HMIS) to collaborate, share insights, and strengthen services.

HMIS
The Homeless Management Information System (HMIS) is a secure database used by homeless service providers to collect and manage information about the people they serve. It helps agencies coordinate care, track services and housing outcomes, and ensure individuals and families receive the support they need.
By using HMIS, communities can better understand local needs, improve service delivery, and report accurate data to funders such as the U.S. Department of Housing and Urban Development (HUD).

Heart to Home Coordinated Entry
Coordinated Entry is how our community prioritizes access to housing and services. Through Heart to Home, individuals are assessed and prioritized so those with the greatest needs can be matched to available housing and support.

TX-604 Waco/McLennan County Continuum of Care
A Continuum of Care (CoC) is the local system that coordinates housing and services to help people move out of homelessness and into stable housing. It is supported in part by funding from the U.S. Department of Housing and Urban Development (HUD), which works alongside other federal, state, and local funding sources. In the Heart of Texas, the CoC serves McLennan, Bosque, Hill, Limestone, Freestone, and Falls counties.
